Leave of Absence

A leave of absence is a period of time when an employee is allowed to take extended time off from their job, usually for significant personal reasons like medical issues, family emergencies, childbirth, military service, or further education, while still maintaining their employment status

If you are going to be on a Medical Leave of Absence (a personal leave does not qualify) for 30 consecutive days or longer, you can request to have your Union dues suspended.

Follow these steps:

  1. When you are aware that you are entering a MLOA for more than 30 consecutive days, please submit a Suspend Dues Form located below.

 

 

*After you submit the form, please be sure to confirm that you want to submit the form through the link that is emailed after you submit it. Click on the link that says "Confirm my email." We won't receive your completed form if you skip this step. After you click confirm, we will receive your form and you will receive a copy via email.

  2. When you have a return to work date, please submit a Reinstate Dues Form located below. 

It is IMPORTANT that you make sure that Union dues deductions restart when you return to work, as owing dues can result in a membership suspension. 
If you don't see deductions within the next couple of weeks of calling us or if you have any questions, please call the Union office at 714-502-0220.
